Post-traumatic cerebral venous thrombosis (ptCVT) is a rare complication of head trauma, however the recent awareness of it has led to the multiplication of diagnosis, and we report a case of ptCVT of the superior sagittal sinus. This case highlights some of the common factors to ptCVT described in the literature, including the existence of associated skull fractures, the latency between trauma, the thrombus formation, and its extent along the sinus. The prognosis remains unpredictable and there is no therapeutic consensus; however, the evolution seems to depend on the age of the patient and the occlusive, non-occlusive CVT type as well as a delayed diagnosis and management. Clinical case; A 4-year-old boy fell from a height of 3 meters he sustained a head trauma, initially conscious with GCS score of 15, he had no signs of intracranial hypertension. Standard radiographs show fractures of the occipital bone. Few hours later, he lost consciousness and had seizures. Admitted in the ICU he regained consciousness but complained of headaches and vomited several times. A CTscan shows a double occipital fractures, straddling the superior sagittal sinus Fig 1, associated with a spontaneously hyperdense sinus (DELTA sign) Fig 2. CT angiography shows a non-occlusive thrombosis of the superior sagittal sinus extended over 80 mm Fig 3. The patient fully recovered within 48 hours without initiation of anticoagulation treatment. Explorations of coagulation profile were unremarkable. Further imaging performed one year later shown complete recanalization of the superior sagittal sinus as well as a fractures healing.

Sofiane Barama is the Neurosurgeon at surgery department Lakhdaria Hospital Bouira Algeria since 2015 to till date. He obtained the Neurosurgery residency program from Neurosurgery department of Salim Zemirli Hospital Algiers Algeria in 2009- 2015. He completed the MD degree at Algiers Faculty of Medicine Algeria in 1999-2007.
