Posterior Subtraction Corpectomy for the management of Traumatic Dorsal and Lumbar Fractures - Two year follow-up experience

Abstract
Objective: To show the experience with the Posterior Subtraction Corpectomy in trauma fractures in the dorsal and Lumbar Spine in an III level hospital of reference in Colombia and to make a systematic review of the literature of the surgical technic. Materials and Methods: We realize a retrospective analysis of the clinic history of the patients that went to the Neurosurgical service between 2016 and 2018 with traumatic fractures in the dorsal and Lumbar Spine. We made an analysis of the pre and postoperative neurological deficit using the ASIA score. We also review surgery times, intraoperative bleeding, and complications. Also, we develop a systematic review of the literature about this surgical technique. Result: A total of 32 patients were taken to surgery, with an average surgical time of 396 minutes; the presurgical ASIA score was B: 50%, C: 47%, E: 3%. The postoperative ASIA score was B: 9%, C: 47%, D: 38%, E, 6%. We obtain a recovery of the previous neurological deficit in 72% of the patients. The average intraoperative bleeding was 1,233 cc. We have 4 complications: 2 hematomas, which require re-intervention, and 2 leakages of cerebrospinal fluid the ones, were managed with a compressive bandage. Conclusion: The Posterior Subtraction Corpectomy requires some skills and from a multidisciplinary team including Neuro Anesthetist and Neurosurgeons Trained in the Spine. It shows a lot of benefits compared with the anterior procedure, which includes less surgical time, less intraoperative bleeding, and less morbimotality.

Juan Manuel Rodriguez Gil is a medical graduate from the Universidad El Rosario, with a specialization in Neurosurgery from the Universidad Del Rosario - Hospital Universitario Mayor - Mederi Universidad Del Rosario - Clínica San Rafael from August 2011 - July 2016. In his career, he has 8 years of experience in patients with brain and low back pain problems. He has taken different courses of important updates throughout my professional career.
