Periodontal prevention during orthodontic treatment

Abstract


Gingivitis and periodontitis arising during orthodontic treatment are common, as can be found in many literature reviews. Several principals have been used to limit this collateral damage, but never with a manual water jet. The protocol we propose here aims to limit periodontal damage that can possibly be caused by orthodontic treatment. This proposed model could be the basis of clinical trials that also include comparison with a control group. Patients during fixed orthodontic therapy could often have an increase in gingival plaque production and population. A recent literature review which considered over 200 scientific articles on this subject have confirmed this, especially at the beginning of orthodontic therapy. The concentration of periodontal bacteria increases when therapy is started, but that tends to decrease over the following months. The most compromised element during orthodontic therapy is the central incisor. Literature studies identified four periodontal species; Porphiromonas Gingivalis, Prevotella Intermedia, Aggregatibacter, Tanerella Forsythensys. However, no data was found on the globality of bacterial species.The use of manual/ electric toothbrushes, interdental brushes and superfluous, while indispensable, are not sufficient tools to perform good home oral hygiene. For example, they don’t allow for the maintenance of bacterial plaque at an acceptable level for orthodontic patients.
