Leiomyosarcoma of porto systemic circulation: Review of literature

Abstract
Background: Leiomyosarcomas arising from the portal/mesenteric venous system are very rare tumors, and only a few cases have been reported in the global literature. We hereby present management and follow-up on our experience with leiomyosarcoma of superior mesenteric vein and compare it with the results obtained by other published literature.
Methods: A Medline search was performed to review the literature for all reports on leiomyosarcoma arising from portal/mesenteric venous system in the English language between 1965 and 2012, and the reference lists provided by those articles were further explored. Data were retrospectively reviewed and information was tabulated for year of publication, age, sex, site of tumor, surgery performed, adjuvant treatment if given, follow up and outcome.
Results: Nine reports only could be collected of patients with leiomyosarcoma of portomesentric system. Patients who underwent surgical management had better outcomes and lower mortality rates compared with no surgical treatment. Most of them developed metastasis but had a reasonable outcome after aggressive surgical management of metastasis.
Conclusions: The relative scarcity of these lesions, histologic diversity, and their presentation at various anatomic sites has made full characterization and understanding of this disease process difficult. Aggressive surgery is the primary therapy and main prognostic factor.
