Idiopathic nephrotic syndrome in 420 children

Abstract
420 patients with idiopathic nephrotic syndrome over the age of one year followed during the period 1994 to 2015. 70% of patients were over 4 years old at the time of diagnosis. The sex ratio is 1.95. The high blood pressure is present immediately in 90 patients (21.4%). 300 Patients are steroid sensitive (71.4%), with 60% of steroid-dependence (250 patients), among them 141 (33.6%) have frequent relapses. The cure rate is 11.9%. We have 120 cases (28.6%) of steroid resistant nephrotic syndrome. 94 patients have a kidney biopsy. It revealed normal morphology in 3 patients, MCD in 35 patients, FSGS in 50 patients and diffuse glomerulosclerosis in 6 patients. Immunosuppressive drugs were used in 86 steroid-resistant patients and 129 steroiddependent patients. 70 patients (16.6%) progressed to chronic renal failure. We have 200 cases of disease complications and 152 cases of complications related to treatment. 14 patients were dialysed. Two patients have a kidney transplant. 15 patients died. However, linear growth of steroid dependent patients followed for more than 10 years is at the limit of normal. Conclusion: In our country, there is a clear male predominance in nephrotic syndrome in children. The high blood pressure is present from the outset in almost a quarter of the cases. The steroid dependence is the most common evolution. We have 28.6% of steroid-resistance with several histological lesions. The disease evolves towards chronic renal failure in 17% of cases and long-term growth seems normal despite long-term corticosteroid therapy.

Lamia Oukrif has completed her medical studies at the age of 26 years from ALGIERS 1 University and Postdoctoral Studies from School of Medicine, Benyoucef Benkhedda University, Algeria. She has submitted her thesis in 2015. She is the head of the unit of pediatric nephrology in pediatric B division in hospital University center Isaad Hassani Beni Messous Algiers. She has published more than 7 papers in reputed journals.
