Abstract

Introduction: Among the non-surgical treatments for basal cell carcinoma (BCC) is interferon, although the main guidelines do not recommend it. More evidences on its effectiveness and safety are needed. Objective: To describe the effectiveness and safety of HeberFERON in the long term in the treatment of facial basal cell carcinoma in a context of usual medical practice in primary health care. Methods: This was an observational case series design. Patients with facial BCC, not amenable to surgical treatment. Perilesional a combination of interferon alpha-2b and gamma was administered for 3 weeks. Patients were Follow-up for 5 years. The main outcomes variables were cure, recurrence, the appearance of a second tumor, and adverse effects. Results: 210 patients were included. After treatment, 156 (76%) patients (95% CI 68.5% to 80.7%) were cured. Those with partial response and stable disease had their tumors removed and declared cured. All cured patients were followed up for 5 years, 19 dropped out. Survival analysis showed that they developed a recurrence at 55.9 months (95% CI 54.3 to 57.5) and a second CBC at 52.4 months (95% CI 50.2 to 54.6). Tumor size, previous treatment, age, and the presence of multiple lesions were associated with outcomes. Adverse effects were frequent but not serious and allowed the treatment to be completed. Conclusions: The combination of interferon alpha-2b and gamma is comparable to other non-surgical therapies for BCC. It may be the firstline non-surgical treatment for facial BCC and pre-surgical treatment in primary care.
