VT ablation of automatic left ventricular pourkjnie tachycardia. A 39 year-old man with no history of heart disease and no significant comorbidity was admitted to our department for intense palpitations with dyspnea and chest pain. The electrocardiogram revealed wide QRS tachycardia with right bundle branch block and extreme left axis deviation with ventricular rate of 230 bpm. No response to repeated bolus of adenosine and verapamil infusion. In sinus rhythm during ablation has highlighted the presence of the potential of Purkjnie located in the left inferior-septal area of left ventricle. Ablation in this site during tachycardia was effective to restore sinus rhythm without arrhythmia inducibility.
